Or you can download a referral form here (Right-click to save the application form to your computer to allow you to complete and send it in by email or post) and either fax the completed form to 0161-764-0888, or e-mail it back to: info@rebuildbury.org.uk
Please note that we are very busy, but you can normally get an appointment for your client within a week. If it is an emergency, we will do our best to accommodate your client as soon as we can.
